Study and understand patient's condition and provide treatment and care according to the direction of Nursing Manager and ensure quality of service and customer satisfaction in a Home Care environment.
Clinical Practice and decision-making
• Assess, plan, implement and evaluate patient care, and make changes as necessary.
• Provide high standard of holistic care to patients, promoting their equality, privacy, confidentiality and dignity at all times.
• Assist with patient hygiene, mobility, physical comfort, eating and drinking.
• Observing and reporting specific changes.
• Undertake and accurately record clinical observations
• Use correct procedures for barrier nursing
• Perform basic life support
• Safely administer prescribed medication and monitor effects
• Ensure documentation is accurate and up to date
• Ensure patients' views are taken into account in the decision-making process
Quality Care
• Demonstrate an inquiring approach towards patient care.
• Educate patients and relatives where appropriate.
• Trust duties and responsibilities and carry out all responsibilities with due regard
• Ensure that all duties are carried out to the highest possible standard, and in accordance with current quality initiatives within the area of work.
• Demonstrate evidence-based practice.
Health and Safety
• Be aware of the responsibility of the staff to maintain a safe and healthy environment for patients.
• Undertake patient moving and handling using a safe and correct procedure
Professional development and training
• Attend the training sessions conducted by QAM
• Participate as appropriate in the quality maintenance program.
• Actively participate in team meetings
• Develop an understanding of clinical supervision programs.
• Improve clinical practices through reflection
• Identify and pursue own educational and professional development
