Job Summary: To provide nursing care that is safe, cost effective and of the highest quality in accordance with the evidence, professional standards, Organizational Standards and Policy and Procedure. The nurse works in partnership with patients and their families and collaborates with the multidisciplinary team to deliver effective care. Standards of Care: 1. Completes timely systematic holistic assessments to determine actual and high risk problems. 2. Analyses assessment data and determines, verifies, prioritizes and documents Nursing Diagnoses and outcomes. 3. In partnership with the person, develops an individualized plan of care to achieve the Desire/outcomes. 4. Implements and co-ordinates the interventions to deliver the plan of care. 5. Evaluates and systematically records progress toward attainment of desired outcomes and revise the plan of care as necessary. 6.Consistently practices in accordance with and assists others in relevant legislation, codes and regulations, area specific clinical skills, technology and practice.complies with NCNZ and MCH policies, procedures and standards of practice when making decision regarding delegation and direction.Consistently ensures that clients’ rights are upheld by providing culturally appropriate Information and communication, to enable clients to make an informed choice. 7.Consistently applies ethical principles in own nursing practice, seeking assistance when necessary.Consistently demonstrates skills in advocating for the client, when client requests support or has limited abilities in decision making. 8. Consistently promotes a culturally safe environment for all that enables client independence quality of life and well being. Able to identify, report and remove sources of potential harm and risk within area of practice.
